Processing a Death Certificate

Once the certificate is filled out and reviewed by the family, it must be signed by the funeral director, embalmer (if applicable), and a physician. This process is either done electronically or on paper if the physician is not using the electronic process.

You must pay by check or money order payable to the NYC Department of Health and Mental Hygiene. Cash isn't accepted. It takes 6 to 8 weeks to process the order. Processing time does not start until Vital Records receives your application.
